Soooo many favorites, but off the top of my head real quick here are three. These all have h/c themes:

"We Defy Augury" by blcwriter on live journal

"The List"' by jenny_wren is also a good one on AO3.

"Lazarus" by Ceres_Libera is excellent and part of her "Switch" 'verse on AO3. It's a xenopolycythemia fic.

Don't have the links off hand, but they're all easy to find.

The Basics of Primary Ed 'verse (http://archiveofourown.org/series/45901) by caera1996.
Massive work containing three 40-odd chapter novels and many shorter fics.
Also a fic in the 'verse titled Crash (http://caera1996.livejournal.com/?skip=10&tag=the%20basics%20of%20primary%20ed.) on her LJ page that hasn't made it onto AO3.
It's an AU, which I don't normally like, but this drew me in completely. It's what converted me to Kirk/McCoy - you must read it!!!

The second fic is part of the series, it comes really late - well after the first three long ones. Should've also told you the third long one is still a WIP but gets updated every 2 - 3 months.
